Eligibility Checklist for CCP :

1) Does the client already have a case manager?[1]

  If yes….not eligible
  If no…go to question 2

2) Does the client have either one or more of: a physical disability, psychiatric disability, acquired brain injury, cognitive disorder or intellectual disability? OR are they frail aged?

  If no….not eligible
  If yes…go to question 3

3) Does their disability or frailty impact upon their daily living skills (one or more
  of…personal care, house cleaning, shopping, banking, use of transport, remembering and 
  getting to appointments) to the extent that they will need services to address this?

  If no….not eligible
  If yes…go to question 4

[1] co case management can occur in some cases e.g. with mental health services who will not be addressing any other aspect of the client's support needs. Secondary consultation will always be explored prior to engaging in co-case management.

4) Does the client need assistance accessing these support services or maintaining them à need a case manager?

  If no…CCP can provide info and phone numbers
  If yes…client is eligible for assessment for CCP's case management services

  4a) What is the barrier to accessing the services? (e.g. client's ability to make informed choices,
  client's ability to identify their own needs…)
  4b) What issue(s) impact upon the client's ability to maintain services?
