Merri Outreach Support Service recognises that people who are experiencing problems with homelessness have complex and individual needs.

Merri Outreach Support Service is able to provide:

• short term support to people in immediate housing crisis
• medium term support to people in transitional housing programs and
• long term support to people in public housing.

Merri Outreach Support Service achieves this by being a multi-program agency that provides support to people at all stages along this continuum.

	North West Regional Children's Resource Program (NWRCRP) Developed to assist the homelessness sector in identifying and addressing the needs of children experiencing homelessness. • Read More
	Transitional Outreach Support Team (TOST) Provides support to single women and men and to families who are homeless, or at risk of homelessness, in both an office based and outreach case management capacity within a short to medium term plan. • Read More.
	Community Connections Program (CCP) The Community Connections program provides a response to individuals who are eligible to receive Home and Community Care Services, have complex care needs, and are homeless or at risk of homelessness. • Read More
	Crisis Response Program (CRP) A short-term program that provides case management and support to people experiencing homelessness or at risk of experiencing homelessness. • Read More
