Merri Outreach Support Service is a community based organisation that has been actively assisting men, women and children experiencing homelessness or at risk of homelessness since 1989. The service strives to work together with people using the service to assist them to overcome their personal issues and barriers to social, emotional and financial independence.

Merri Outreach Support Service is an innovative and progressive community service organisation which supports marginalised men, women and children who experience homelessness or housing vulnerability. The Agency delivers services from offices in Broadmeadows, Northcote and Brunswick. These services are either Home and Community Care (HACC) or Homelessness Support (SAAP) funded. Applications are invited for 2 full-time positions available in our Children’s teams based at Broadmeadows.

Outreach Case Manager (0.6 EFT maternity leave position) Click here for job description

A maternity leave 3 days per week vacancy exists with our Community Connections (CCP)/ Housing Support for the Aged team based at our Northcote office available to 4/3/2013. The core function of the position is to provide case management and outreach to older individuals who experience homelessness or housing vulnerability and have complex unmet health care needs. Case management is delivered using a team case management model. The majority of contact with consumers occurs in their accommodation setting. Care coordination is a key component of the role.

Children’s Resource Worker (1.0 EFT) Click here for position description

The Children’s Resource Worker position sits within the 2 worker N&W Regional Children’s Resource Program, alongside the Coordinator. The role involves networking with homelessness and family violence assistance services. The program’s primary aim is to advocate and improve outcomes for children in families who are homeless and/or experience family violence. This is achieved by providing a systemic regional response; advocating for structural reform; improving resources for accompanying children; highlighting service gaps; improving access to services; provision of secondary consultation; administering brokerage; participation in forums and provision of training around children and homelessness to workers. Links to MOSS teams are important. The position is paid as SACS S/W 3, year 3 with car.

Children’s Group Worker/Case Manager (1.0EFT) Click here for position description

The Children’s Group Worker/Case Manager is currently funded to 30 June 2013 and sits within Bright Futures - Children’s Specialist Support Service, an exciting service developed to meet the needs of children from families impacted by homelessness across the N&W Region. The service model includes assessment, enhanced case management and therapeutic group work involving close collaboration with other agencies.  The role comprises a mix of group work and case management. Applicants should be skilled in group work and case work with children. The role requires good assessment skills, ability to work collaboratively, referral knowledge and participation in service promotion. The position is paid as SACS S/W 1, year 7 with car.
