Community Connections Program (CCP)

The Community Connections program provides a response to individuals who are eligible to receive Home and Community Care Services, have complex care needs and are homeless or at risk of homelessness.

The Community Connections Team provides services to frail aged individuals, younger people with disabilities and their carers . The overall objective of the Community Connections Team is to maximise the independence of individuals, ensure access to services, and to prevent premature or inappropriate admission to long term residential care.

Eligibility Criteria

  • Individuals need to have been assessed as needing assistance with the activities of daily living such as dressing, preparing meals, house cleaning, shopping, nursing or personal care
  • People with disabilities of all ages are in the HACC target group. It should be noted that for younger people with a disability, specific purpose programs such as, disability rehabilitation, Department of Human Service disability intake and response and employment support might be more appropriate than HACC services and in such cases CCP will be active in directing referrals to a more appropriate service
  • The Community Connections Program has an active role in assisting individuals to access accommodation appropriate to their needs by accessing community resources.
  • Individuals who have multiple complex care needs and few links to services or links at risk of deteriorating.

Services

  • Utilises an assertive outreach model in accessing potential clients Provides assessment of need and coordinates service response to people.
  • Provides short term case-management to establish links for individuals to appropriate community based services and supports.
  • Supports and maintains maximum independence for people in low cost accommodation, those who are homeless, or at risk of homelessness.
  • Individuals requiring long term support will be referred to an appropriate service

Catchment

  • CCP provide services to residents of Moreland, Darebin, Banyule, Whittlesea and Nillumbik. Residents of other Local Government Areas will be advised of the contact details for their local Community Connections Program

Referral

  • Individuals can self refer to CCP for assessment
  • If a service believes an individual is eligible for assessment, the service provider should complete the CCP referral form. Based on information on the referral form CCP will either an individual, seek further information to clarify eligibility. If the referral is deemed inappropriate the referrer will be directed to a more appropriate service
  • An eligibility checklist and Referral Form are attached to this document
